LAP-BAND® Personalized Patient ID Card
The LAP-BAND® System Patient ID Card is a standard credit card-sized card that fits into your wallet so you can use it at restaurants to request small portions, travel, and visit the doctor. It includes essential information about your LAP-BAND® System requirements, health care and more.
This handy ID Card is available only through My LAP-BAND® Journey. It’s useful through every stage of your life-changing journey. And as a member you’ll have access to email reminders, tips for diet and exercise, tools to help you track your progress and more.
To receive your permanent patient ID card, enter the 7-digit number found on the lower right hand corner on the back of your temporary ID card. The temporary ID card is included in your LAP-BAND® System Surgery Kit provided by your surgeon’s office:
Don't have the temporary ID number?
If you don't have your temporary patient ID card with the 7-digit code, ask your surgeon for a LAP-BAND® System Surgery Kit. This packet from Allergan is filled with resources you will need during your post-surgery LAP-BAND® System Journey and includes the temporary ID card.
If you have no means of contacting your surgeon, please call 1-800-LAP-BAND, option 6, for assistance. You will be asked to answer a series of questions relating to your procedure, your surgeon, and your band.
