LAP-BAND® Personalized Patient ID Card

Thank you for requesting your personalized, permanent LAP-BAND® System Patient ID Card - a wallet card to keep for eating out and requesting small portions, travel or visits to the doctor. It includes essential information about your LAP-BAND® System meal restrictions, health care, and more.

To receive your permanent patient ID card, enter the 7-digit number found on the lower right hand corner on the back of your temporary ID card. The temporary ID card is included in your LAP-BAND® System Surgery Kit provided by your surgeon’s office:
